509-J Sports Physicals

As fall approaches and students in the 509J school district gear up for fall sports – Warm Springs IHS reminds families that the state of Oregon require annual physicals prior to participating in any school sport.

You can call to schedule for an appointment for a sports physical for July 26^th or September 1^st.

Parent/Guardian attendance is required at the appointment.  Call 541-553-2610 and ask to schedule a sports physical.
